Refer a Young Person

This is the online referral form to the Kookaburra Kids programs.

Please take your time to read our referral criteria and complete the details to the best of your knowledge. Should you require any assistance when completing this form, please phone your local Kookaburra Kids office on 1300 566 525.

Once the form is completed one of our in-house Clinicians will be in contact with the family via phone, to finalise the process and document the best ways to support the young person/s while on program. This contact could take up to 30 days. The referrer should receive a confirmation email once completing the referral.

At Kookaburra Kids, we are committed to protecting the privacy and confidentiality of all referral information. This statement outlines how we manage the personal information provided to us through referrals and the purposes for which it may be used.

Use of Referral Information
The information provided to Kookaburra Kids through referrals is primarily used for reporting and research purposes. This helps us to improve our services and understand the needs of the young people and families we support. Your personal information will be handled with the utmost care and confidentiality.

Sharing of Referral Information
In some instances, your referral information may be shared with other organisations. This is done to facilitate follow-on referrals (with your approval/knowledge) and ensure that you or your young person receives the appropriate support and services. We only share information with trusted partners and other relevant organisations when it is necessary for providing further assistance and support.

Wellways Supported Programs
If the referral pertains to a Wellways supported program, and you consent to participating in this program, you are also consenting to Wellways receiving your referral information. This automatic consent is necessary to provide you access to additional supports through the Carers Gateway Australia. Wellways will use this information to ensure you receive comprehensive care and assistance tailored to your needs. This is indicated by the title of the program in the booking system ie. STATE (Location) Camp Name (CORE) (Wellways).

You have the right to access your personal information and request corrections if necessary.
